CORRECTION: SOW 4.3 & 7. corrections are made to SOW to reflect actual POP in section 7 as stated in this solicitation & 4.3 to state insurance liability requirements (state minimum requirement). Furthermore, bottom of this discription is to have quotes emailed Contracting Officer prior to the expiration date of this notice.



This is a Combined Synopsis/Solicitation for Transportation of medical patiences to and from medical appointments. This public notice of this RFQ is prepared in accordance with the format in FAR Subpart 12.6, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures and FAR Part 17.2 Options , as supplemented with additional information included in this notice. This announcement constitutes only this solicitation.



The Solicitation [60100-PR-24-RFQ00001] is issued as a Request for Quotation (RFQ). Submit written offers only, oral offers will not be accepted. This Combined Synopsis/Solicitation requires registration with the System for Award Management (SAM) at the time a quote is submitted, excluding outlined in FAR 4.1102(a). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-05 effective 09/23/2023. The complete text of any of the clauses and provisions may be accessed in full text at https://www.acquisition.gov/browse/index/far.



This procurement is a 100% Total Small Business Set-Aside and the associated North American Industry Classification System (NAICS) Code is 624120 - with size standard $15.0M & 485999 - with size standard $19.0M ,. Product Service Code: V122. ****Multiple NAICS apply to this service criteria****



Location of Service: USCG Academy, 15 Mohegan Avenue, New London, CT 06320



Discription Requirements:



Per Statement of Work (SOW), Medical transportation service. Members (patients) are to be picked up and brought to their destination and back to attend appointment(s). Vehicles like vans, sedans, sport-utility type to transport patients. Smoke free vehicles for patients. Initiatial pick-ups shall be one way. Pick-ups for return rides form appointments shall not exceed a wait time more than 30-minutes. Pick-ups may have multiple members at an initial pick-up. Service requirements MUST meet all expressed in SOW attachment 3.c_Statement of Work - 2023.



POP: 02 CLINs (one representing each year)



Base year: 01-November-2023 thru 31-October-2024



Option Year 1: 01-Novemember-2024 thru 31-October-2025



Service Contract Act Wage Determination No. WD 2015-4111 (Rev.-25) was first posted on www.sam.gov "Wage Determinations", applies to this acquisition and will be incorporated into the resulting contract. The contractor shall identify the Wage Category in their offer.



Evaluation factors: Quotes shall include Monthly Firm Fixed Price (FFP) amount multiplied by 12 to reflect each year as the extended cost. The total cost shall be Firm Fixed Price for 2-years.



The initial award will be based on the following criteria: Lowest Price Technically Acceptable, availability to perform service.

Offerors Representations and Certifications: A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically). An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/. All Offerors shall be registered and current in the System for Award Management (SAM) database.
