The Department of Public Health (DPH) TB, HIV, STD, and Viral Hepatitis Program is seeking a contractor to coordinate, facilitate and evaluate the activities of the Connecticut HIV Planning Consortium (CHPC). The Department is federally mandated by the Centers for Disease Control and Prevention (CDC) and the Health Resources and Services Administration (HRSA) to convene a statewide HIV Planning Group (HPG) to develop a comprehensive and integrated plan for HIV care and prevention service delivery that meets the needs of people living with HIV and those most at risk for acquiring it. The Integrated Plan will be expanded beyond HIV to include Viral Hepatitis Elimination and Substance Use Disorder.

The purpose of this RFP is to solicit bids from qualified vendors to organize and manage the operations of a statewide community planning group as well as oversee the strategic plan process. Please read the attached documents for more information.
