The University of Connecticut Health Center (UConn Health) is requesting proposals from qualified organizations to provide community case management services to adoptive and guardianship families. The successful proposer(s) will assign a dedicated staff member to support the AAP by completing in-home family assessments, providing family support, assisting families' access of community supports, increasing community awareness around the needs of adoptive and guardianship families/children, developing community resources, and marketing the AAP. It is the intent of the AAP to have this work performed by four (4) half-time positions. The assigned contracter(s) will accept referrals from the greater Bridgeport, Waterbury, New Haven and Eastern Regional areas.
