The Connecticut Department of Public Health (DPH), Chronic Diseases Unit is seeking proposals from Connecticut public and private organizations and community-based agencies to provide training programs for the Alzheimer’s Disease and Related Dementias (ADRD) Program for medical professionals and community professionals. An anticipated total of approximately $100,000 annually for a total of $500,000 of federal Centers for Disease Control and Prevention (CDC)
ADRD funds are available to support this project.
Component 1: Training for Medical Professionals will be supported by $50,000 annually for 5 funding periods and Component 2: Training for Community professionals will be supported by $50,000 annually for 5 funding periods. Applicants may apply for one or both components of funding; however, if applying
for both components, a separate scope of services and budget must be provided.
