The City of Norwalk is requesting proposals for qualified Contractors within the City of Norwalk or a 5 mile

radius to provide professional Occupational Health Services beginning the effective date of the fully

executed agreement for services relating to pre-employment, post-accident, return to work (fitness for

duty) physical exams, drug and alcohol testing, both DOT and Non-DOT and physical testing as required by

City and Fire Department. The requirements of this project are outlined in greater detail under Section 2

Scope of Work/Project Specifications.
